Sometimes it can be helpful to let hospital staff know about your visual impairment and any support or adjustments you may need. This can be especially important if you are unwell or staying in hospital, as repeatedly explaining things can be tiring.
We have created a simple document that you can display during your hospital stay to help staff understand your needs. It can be placed near your bed or attached to the end of the bed, if that feels helpful for you.
You can print and laminate the document using the links below. Versions are available with both white and yellow backgrounds. If you are unable to print it yourself, we can print and laminate the document for you.
